The basic function of the CreativeChoices Personal Caregiver is to provide personalized assistance, companion care and/or escort services to residents. The Personal Caregiver lends support to individuals in completing their daily living tasks and helps to promote healthy socialization and quality of life. The Personal Caregiver provides support within the guidelines specified. This is a full-time, dayshift position.
The employee must be able to perform each essential function effectively to be successful in this position.
1. Abides by the CreativeChoices Protocols for Resident Care (see separate attachment).
2. Delivers personal care assistance in a kind, compassionate manner with respect for client dignity and individuality.
3. Organizes and completes assigned duties for clients in a timely manner.
4. Performs services outlined in the service request, informing supervisor of additional services or changes to service times requested by resident.
5. Provides assistance in a manner that protects and promotes resident rights, dignity, privacy, confidentiality, self-determination and active participation.
6. Assists with resident’s daily living tasks and lends support for personal adjustment.
7. Observes the resident’s overall physical/mental condition and behavior and notifies the CreativeChoices Director if there are significant changes in the resident’s status.
